Policy FAQs 

As well as your policy document, there are other documents that form part of your health insurance policy. This includes your application form, your membership certificate, and any health insurance medical declaration you may have completed, together with:

• The eligibility criteria
• The list of unapproved healthcare services
• The list of Prostheses and Specialised Equipment
• The list of policy variations.

These lists will be available on the Southern Cross website. They may be updated from time to time and it’s your responsibility to check them.
