Why do you need to collect so much health information in my application

To give you certainty of what we will cover under your policy, we need you to give us as much information about your health (past and present) to the best of your knowledge for yourself and any members included in the application.

This is important because we don’t cover treatment that relates to a pre-existing condition and we only know what you tell us about your health. When we receive your application, we decide what we will (or will not) offer cover for based on the information you give us.

Your membership certificate shows what cover you and any family members on your policy have for qualifying pre-existing conditions. You can view this in My Southern Cross.

We may decline cover for any pre-existing conditions that weren't disclosed in your application and which are discovered later. This means we may amend the terms of your policy and will decline cover for any affected claims.
